Researchers analyzed real-world data from nearly 170,000 people with Type 1 diabetes using the MiniMed 780G automated insulin delivery system across Europe, the Middle East, and Africa over one year. Their goal was to understand how different low-glucose alert (LA) threshold settings—the blood sugar level at which your pump sends you a warning—affected actual glucose control and hypoglycemia outcomes.

The Majority Use Alerts—and They Work

Ninety-six percent of users had low-glucose alerts turned on. About one-third also used predictive alerts, which warn you before your glucose is predicted to drop too low. This widespread adoption suggests that people find alerts helpful, even when the pump is automatically reducing insulin delivery to prevent lows.

Most Users Stay Safe Across Different Alert Settings

Regardless of which alert threshold they chose, the vast majority of users spent very little time with dangerously low blood sugar. Time spent below 54 mg/dL and below 70 mg/dL remained low and within recommended targets across all alert threshold groups. This held true for children and teenagers, and overnight as well. More than 82 percent of users with a threshold set to 80 mg/dL achieved hypoglycemia targets, and over 90 percent of users with lower thresholds did so.

What the Evidence Suggests for Your Own Settings

The data suggests that a low-glucose alert threshold below 60 mg/dL is associated with the best overall glucose control, more time in healthy range, and less high blood sugar. However, this isn't a one-size-fits-all answer. A threshold between 60 and 70 mg/dL may offer a practical middle ground if you're particularly worried about lows or find frequent alerts bothersome.
