What Matters More for Heart Health in Type 1 Diabetes: Liver Fat or Body Composition?

Researchers have known for years that fatty liver disease increases heart risk in adults. But whether the same is true for young people with Type 1 diabetes hasn't been clear. This study looked directly at that question.

What the Study Found

Scientists studied 49 teenagers with Type 1 diabetes (average age 16 years) and measured three things: how much fat was in their livers using a special MRI scan, their body composition (muscle versus fat) using a bone density scanner, and early changes in their carotid arteries (large blood vessels in the neck) using ultrasound.

The results were surprising. Liver fat levels were very low across the group—in fact, none of the teenagers had enough liver fat to be diagnosed with fatty liver disease. When researchers looked at what was actually linked to early artery changes, lean body mass (muscle) stood out as the main factor. Liver fat, insulin sensitivity, and how long someone had been living with diabetes were not independently associated with these vessel changes.

The Belly Fat Puzzle

The study also found that waist circumference (a measure of belly fat) was strongly linked to insulin resistance. However, belly fat was not directly associated with the early vessel changes being measured. This suggests that while central fat and metabolic dysfunction are connected, they may not translate directly to early structural changes in blood vessels in young people with Type 1 diabetes.

What This Means

Because this was a small, cross-sectional study (a snapshot in time), the findings apply most directly to teenagers in the sub-steatotic range—meaning those without significant liver fat. The researchers note that their results likely reflect normal physiologic growth: teenagers naturally develop muscle and change body composition as they mature, and these changes appeared to be the strongest driver of early vascular remodeling.

The study does not mean that liver health or weight management are unimportant for people with Type 1 diabetes. Rather, it suggests that in young people without fatty liver disease, factors related to normal development and muscle growth may be more relevant to early heart vessel changes than liver fat itself. More research in larger groups, and over time, is needed to confirm these findings and understand their long-term significance.
