Researchers used data from the Global Burden of Disease 2021 study, which tracks health conditions across countries and populations. They looked at new cases, deaths, existing cases, and overall disease burden, breaking down results by age groups and by countries' economic development levels.
Mixed Trends: Fewer Deaths, More New Cases
The study found conflicting patterns in early-onset CKD. The good news: death rates have been declining since 1990, with an average drop of about 0.87% per year. This suggests that treatment and medical care have improved for young people with kidney disease.
However, the number of new CKD cases among children and young adults has been rising—though at a slower pace, around 0.31% per year. Overall, researchers estimated there were 41.79 million cases globally by 2021.
These opposite trends mean that more young people are being diagnosed with CKD, but fewer are dying from it. This points to better survival but also raises questions about what is driving increased incidence.
Who Is Most Affected
The disease shows distinct patterns across different ages. Children under 5 years old account for nearly half (45.58%) of all new cases of early-onset CKD. Young adults aged 20 to 24 carry the highest disease burden in terms of disability and life years lost (32.32% of DALYs).
The study also found that males experience higher mortality from early-onset CKD than females, a difference that was consistent across age groups.
Stark Global Inequalities
Early-onset CKD does not affect all regions equally. Death rates from kidney disease are lowest in wealthy countries and highest in low- and medium-income regions. Conversely, new cases are rising most sharply in lower-income areas.
This pattern suggests that access to diagnosis, treatment, and supportive care varies significantly around the world. Wealthier nations appear better equipped to keep young people with CKD alive, while lower-income countries face both higher incidence and worse outcomes.
Looking Ahead
Using statistical models, researchers projected early-onset CKD trends through 2050. The findings underscore the need for prevention strategies and equitable access to kidney care worldwide, particularly in regions where both incidence and mortality remain high.
