Results for Type 1 Diabetes

For people with Type 1 diabetes, simplified bolusing and carbohydrate counting produced very similar results. Those using simplified bolusing spent about 2.5% less time in the target blood sugar range, had nearly identical time below 70 mg/dL, and had a mean glucose about 5 mg/dL higher.

The researchers concluded these differences were not clinically meaningful—a finding they called 'non-inferior,' meaning simplified bolusing worked about as well as traditional carb counting. This held true across different age groups and in all four countries studied.

What This Means for Daily Life

Carbohydrate counting is effective but requires effort: measuring portions, reading labels, and calculating insulin doses. Simplified bolusing cuts this down by letting users pick from a pre-set list of their favorite meals.

The study suggests this trade-off is worth making. Users could reduce their daily cognitive load without sacrificing blood sugar control. However, blood sugar results were slightly better with traditional carb counting, so the choice may depend on individual priorities and how much detail each person finds manageable.

What We Still Need to Know

This study used real-world data, which is valuable but has limitations. People chose their own bolusing method, so the groups may have differed in ways beyond just their approach. Longer-term outcomes and how this works for people learning to use their pump for the first time would be helpful to explore.
